CentraCare is asking residents in Benton, Sherburne, and Stearns counties to watch for the Central Minnesota Community Health Survey and to please fill it out. The survey is a collaborative project between local public agencies and CentraCare to gather information that will create a more accurate picture of the health and wellness in our communities.
The survey will be mailed in the beginning of April to randomly selected households. The survey is completely confidential. It asks questions about the residents’ access to health care, nutrition, physical activity, tobacco use and other social, physical, and mental health related topics. Data from the survey is valuable information used to understand the current health status of the tri-county region, identify health perceptions and barriers to healthy living, and develop strategies to improve the health of the residents.
